For myself, it's was a varicocele extremely close to the teste itself. Finally, catching cancer early is good - early enough and you may even get to keep that particular teste if it does turn out to be cancer.

I wouldn't say painless. I've had mine for about 15 years now (almost got it removed a few months ago, but chickened out), though it's only about half the size of the one it is attached to. Sometimes it just aches, but getting kicked in the balls is nothing compared to if that cyst gets bumped wrong.
It could result in sperm not being able to get out, but it's ability to get out is already severely impacted by the cyst to begin with. So it's kind of a wash in that regard. The big thing is that unless they remove the epididymis (which means nothing is getting out anymore), the spermatocele can reform.
Ice had an ache in my balls for months, but two times of ultrasound reveals nothing but varicose veins (on the side that doesn't hurt). Varicose veins down there can lower fertility so that's a thing to keep in mind. Other things we might get are parasitic twins (Google at your own risk😅) or hernias. Regardless: ALWAYS get checked if you have aches and/or a lump. Survival rate is very very high when caught early. Don't delay it, and check yourself regularly
