I'm a registered dietitian specializing in enteral tube feeding with experience working with both the elderly and pediatric patients with dysphagia. I understand where you're coming from but telling people to stop using thickened liquids as blanket advice is damaging and misinformed.
These products should only be used after being evaluated by an SLP (speech language pathologist) that can determine the proper thickness based on a person's current ability to swallow liquids.
Additionally, what's the alternative? People using these products use them because they are choking and often aspirating on normal liquids. If you can't get adequate hydration orally then you would have to resort to enteral tubes and for most people that is a drastic quality of life decrease. For many patients wishing to maintain bodily autonomy, thickened liquids are the preferred alternative.
I am a speech language pathologist. Show me a single study that indicates that use of thickened liquids provides long-term benefit in preventing aspiration pneumonia. One single solitary study. I have 19 that say they don't. 4 that say they lead to worse outcomes. The Frasier freewater protocol has been the leading hydration support for patients with dysphasia for two decades. This is not new information.
I will not provide evidence that it doesn't prevent or even treat aspiration pneumonia because (and you should be happy about this part) I took your comment to heart and did a little research and you are correct. This will actually change how I approach patient care and I will push for more usage of the FFP with my fellow clinicians. However, here's the thing (second comment incoming)
When I looked into the FFP, it's not widely used or even known about by a majority of clinicians outside SLP's. I am a semi-recent RD that completed my Master's and became credentialed in 2019. When I went through school we were still being taught the national dysphagia diet when I was in undergrad in the late 20-teens (yes I know NDD is now defunct). I think more awareness needs to be brought to the FPP. So far it seems like only around 30% ish or less of nurses feel okay administering it.
At 8% ABV? How much/fast are you drinking? Isn't beer relatively filling? Almost everyone I've ever known to get fucked up (self included) got done in by cocktails because they're high-ABV but don't taste like it.
