It depends on the game, I believe, but it doesn't count people who own but have never opened the game. I think for DS, it counts as soon as you enter character creation.
I figured just owned games. I once added up every achievement I earned and every potential achievement across all my games (including those not launched) and I had some 19-20% earned, on average. And I try. Games like TF2, with 500+ achievements after I stopped playing skew my average

Example: Loop Hero. "new Beginning" - Embark on your first expedition. Which is literally press the button to play the game. 97.4% of players have it.

Reminds me, there are also achievements that get added later. Half Life 2 didn't come with any and has 13 now, with the highest at 69%.
