You remove the ball cover, take the old ball out (if there is one), optionally clean the gunk off the rollers with tweezers (those things got nasty, which could cause issues), stick the new ball in the socket, and replace the cover. It wasn't exactly rocket surgery...

People used to steal them from school to use as ammo in pipe guns. You get some pvc pipe, cut the finger off a rubber glove, tape it over the end of the pipe, and pop the mouse ball in the other end. Grab the ball through the finger and stretch back. Release to launch. It's not devastating. But as a bit of juvenile mischief they were good fun.
