What action can be put in place to prevent this happening in future? I dunno... give me less work or more time? No I mean what standard procedure can be put in place to prevent it? The process is plan, code, test, code more, test more... Don't do it wrong is already pretty heavily implied...

What I showed is a joking of the standard procedure. The normal process is 1: Idiot managers decide the coders should program something stupid that no one needs. 2. The programmers program it. 3. The QA department tells everyone it does not work so the programmers need to work on it more. 4 Repeat.
