Corruption, inefficiency, and aiming the resources at the wrong problems, mostly. Plus, kicking anyone out of the program as soon as they had a relapse or minimal success, ensuring that any problems immediately landed them back homeless, and they would go back on a waiting list. That is usually how Washington did it as well from what I saw in Seattle. Just starting with a safe, warm place to park for the night and get washed regularly does wonders for mental health and hireability.
