Emergency brakes probably adjusted wrong. Or maybe someone jumping while it was moving gave just enough cable slack to engage the brakes.
Either way, definitely needs some maintenance done to get everything to proper specs, and I don't blame you for not using it.

Many of those rules are by state laws or by city ordinances I think. We had to have ours inspected once per year tho
