It's a disservice to teenagers to imply that any time a teenager has sex it's rape. Teenagers aren't toddlers. They're developing into adults, and with that comes adult behavior which can be important to development. That's why circumstances and context matter so much. I've met several women who've told me about losing their virginity early and being glad for it. There are also victims. It's a highly variable and personalized thing. It's also why power imbalances are such an issue.

I can assure you that it was taboo for a looooong time before that. Some women got married young, but it was taboo to consummate the marriage before 16-ish (depending on the exact time period and geographic region). They understood that while it was possible to have a child younger than that, it was often fatal to both mother and child, and if it wasn't fatal it would often cause complications that would make future successful pregnancies more difficult.

No; it is left to the discretion of the states. Most picked 16, some picked 18, and a few went with 17. Before 2001, Hawai'i was the only one to have anything lower (14), but it was raised to 16 in that year.
Many states have additional provisos about "close in age exemptions", so some of those 16s might only apply to people no more than 3-5 years older.

To the issue of why it would be state-decided.... Essentially, the rule is that, if the Constitution does not confer a legislative power to the federal government, it's left to the states. It's Amendment 10. To add laws to federal jurisdiction requires amendment to the Constitution, which has a higher threshold than simple majority to get to happen.
