It didn't START OUT as a racial slur.

How offensive a given word can be depends on what's going on socially at the time. Around your grandma's time, this word was suddenly becoming VERY offensive, but depending on where they lived, the idea may not have sunk in yet.

I know that in rural areas at the time, especially in the western states, they were often isolated from current events enough that culture only shifted when someone moved away and then came back with a new perspective.

1 - My mom use to do that, too, until I told her it was really offensive. Took a few times of when she'd say it that I'd automatically correct her by saying 'Brazil nuts' instead for her to stop.
Same with N* rigging instead of just saying tinkered with / worked on.

We grew up with all the N weird sayings and as children we never connected that it was derogatory towards black people (I know, it feels crazy even saying that). We lived in a very small conservative town in Colorado in the 70s & 80s and that was just part of the lingo there. As an adult I recognize just how fucked up that is, it was just how things were said there.

To be honest, I think those words are not inherently bad, but because of how they are associated with people's behavior. Other slurs for sure can be more directly hurtful. But a kid can only see the surface-level stuff. It doesn't know that below, there are much deeper running roots of racism that can resurface at any time. An adult knows that using these words with their connotations even in an innocent context is working to keep those roots alive and close to the surface.
