I studied virology on the Internet so I'm an expert now and this is my theory: most patients have respiratory infections, but in some of them the virus affects the brain instead, without presenting visible symptoms but messing with their cognitive abilities. That would explain so much, including the massive overlap between anti-vaxxer Covid deniers and QAnon crazies.
