I’ve had it at few different old diners. Diced or chopped ham trimmins, mayo, and with veg and herb. Kind like ham mixed with tartar sauce.

God I thought I was old. When I started in the 70s cigs were 55 cents. Lots of tax in PA. I bought horror comics for 45 cents .
